You must include one or more of the following documents with your application: • Last year's federal income tax return
• Unemployment statement
• Social Security benefits letter
• Proof of any other assistance you receive (ie. food stamps, housing etc.)
• Proof of dependents (copy of income tax return or birth certificates) Failure to provide the necessary documents will delay the processing of your applications. If you do not qualify for a discount, you may call our office to discuss payment arrangements at 843-792-2311 or 1-800-598-0624.
